Verification of Agency Return Address on Employee W-2 Statements

Purpose

To request Payroll Offices to verify their agency address which will be used as the return address on employee W-2 statements for 2004.

Background

Prior to 2004, OSC’s address was printed as the return address on employee W-2 statements, except for employees of agencies participating in the W-2 direct mail pilot program. Undeliverable W-2s were returned to OSC, and then forwarded to the appropriate agencies by OSC. Beginning with the 2004 W-2s, the agency return address will now be used for all employee W-2s.

Agency Actions

To confirm that the correct agency address is in the PayServ payroll system, agencies must view the Control-D Report NPAY 529, “Agency W-2 Return Address Report.” Agencies must verify the address listed is correct for all applicable Department IDs. Agencies must send an e-mail to [email protected] by November 10, 2004, confirming that the existing address is correct or notifying OSC of an address change. Please type “W-2 Return Address Reply” on the Subject line of the e-mail.

For agencies that cannot send an e-mail, a fax must be sent to (518) 486-3099.

Note: This address information will be used only to print the agency return address on employee W-2s.

OSC Actions

OSC will update the Agency W-2 Return Address file with the information supplied. A payroll bulletin will be issued at a later date requesting agencies to verify a new Control-D Report NPAY529 to confirm updated information.

Response Deadline

Agencies must respond by November 10, 2004 to ensure that the correct return address is printed on employee W-2s for 2004.
