Purpose
To inform agencies that the Direct Deposit Form for NYS Employees (AC 2772) has been revised to require employees to provide only the last four (4) digits of their Social Security number (SSN)

Background
To comply with personal privacy laws, OSC will now require employees to supply only the last four (4) digits of the SSN on this form

OSC has made available a new search method to help agencies identify employees in PayServ using only the last four (4) digits of the SSN, with or without using the Last Name field.
The revised Direct Deposit Form for NYS Employees (AC 2772) is also available on the Payroll pages of the OSC Web site.
Agency Actions
When an employee submits a new Direct Deposit Form for NYS Employees (AC 2772), agencies should use this new search method:
- Open Direct Deposit in PayServ. Path: Home > Compensate Employees > Maintain Payroll Data (US) > Use >Direct Deposit.
- In the EmplID field, you may do one of the following:
- enter five (5) underscore characters followed by the last four (4) digits of the employee’s SSN.
Example:
- enter five (5) underscore characters followed by the last four (4) digits of the employee’s SSN. In the Last Name field, enter employee's last name and then hit "Enter."
- Example:
- Use the generated list to identify the employee in question and click on the correct employee's name. The Direct Deposit page will appear for entry of new or updated Direct Deposit information.
Agencies may wish to make this revised Direct Deposit form available to employees through links on their agency Web sites and in new employee orientation programs.
Agencies should destroy all previous Direct Deposit enrollment forms (AC 2772).
