To request agencies to review and verify the new updated Control-D Report NPAY 529.

OSC has updated Control-D Report NPAY529 with the information supplied by the agencies.

Agency Actions

Review Control-D Report NPAY 529 and verify that the agency address listed is correct. If the agency address is correct, no further action is needed. The agency address that is listed on the updated NPAY 529 in Control D will be used as the return address for employee W-2 statements for 2005.

Response Deadline

If corrections are needed, agencies must send either an e-mail to [email protected] or a fax to 518-486-3099 to notify OSC of an address change by December 29, 2005.

In the subject line of your e-mail, please type “W-2 Return Address Reply” and your five digit Agency Code number(s).
