| Purpose | To explain a change in name of one health plan and the addition of two new health plans |

| OSC Automatic Processing | At the request of the New York City Benefits Office, OSC has added three new health plans. They are Empire EPO, Empire HMO New York and Empire Health Care New Jersey. The Empire Blue Choice Plan has been eliminated and replaced by Empire EPO. All employees who did not change their plan from Empire Blue Choice to another health plan are now automatically enrolled in Empire EPO and will pay the Empire EPO rates. Detailed information on these changes and rates have been provided by the Employee Benefits Office. |
