I Need to Report a Death »

You can report the death of a member or retiree by phone, email or mail. Please note that we will also need an original, certified death certificate.

Call: 1-866-805-0990 and press 2 and then 1. You will be asked for the member’s retirement or registration number (or Social Security number if you don’t have either account number) and the date of death. You may also be asked for the address and phone numbers of immediate family members who may be beneficiaries.

Email: If you have the member’s retirement, registration or Social Security number, you can email us using our secure contact form. In your email, please be sure to provide your phone number(s) in case we need to call for additional information. Please also provide your mailing address in the body of your email, or in the address field if you don’t know the deceased member’s mailing address. If you are unsure of any of the member’s information, you can enter place holders (for example, you can enter 999-99-9999 if you don’t know their Social Security number).

Mail: You can complete and mail in a Notification of Death form (RS6082) Adobe pdf.

Please note that we will begin processing any death benefits that may be payable once we receive a death certificate.

Cost-of-Living Adjustment (COLA)

  When will I be eligible for COLA?

If you are not yet receiving a COLA payment, you can hear when you will be become eligible by using our automated phone service. Dial 1-866-805-0990, and press 4 for retirees, and then 2 for COLA eligibility.

  How much is my COLA?

If you are already receiving a COLA payment, our automated phone service can tell you your current COLA amount and what the current COLA rate is. Dial 1-866-805-0990, and press 4 for retirees, and then 2 for COLA eligibility. Once COLA payments start, they will continue automatically and your benefit will be adjusted every September. You will receive a letter from us every fall letting you know what the new rate will be.

  Where can I find general information about who is eligible, this year’s rate and how COLA is calculated?

Federal Taxes

  How much is my current withholding?

Use our automated phone service to get your current withholding status. Dial 1-866-805-0990, press 4 for retirees, 5 for Tax-Related Services, and then 2 for Information about Your Current Federal Withholding.

  How do I change my tax deduction?

You can change your tax withholding status at any time by filing a W-4P form Adobe pdf. For help completing a W-4P form, try out the interactive tutorial on our Understanding your W-4P Form page. Our Withholding Tax Calculator can tell you how much will be withheld if you choose a different number of exemptions or status (for example, married with 1 exemption or single with 0 exemptions). You can mail it to the address on the form, or fax it to 518-486-3252.

  How much will be withheld when I submit my W-4P form?

It is up to you to let us know how much you would like withheld from your retirement benefit for federal taxes. If you know what status you want to claim (for example, married with 1 exemption, or single with 0 exemptions) you can find out how much we would withhold by using our Withholding Tax Calculator.

Health Insurance

  I have health insurance questions.

We don’t administer health insurance programs. We may deduct premiums from your monthly pension benefit to pay for your health insurance coverage if your former employer instructs us to do so, but we are not able to tell you why your health insurance deduction may have changed.

Loans

  How much can I borrow?

You can borrow up to 75 percent of your contribution balance, minus any outstanding loan balance. For Tier 1 and 2 members, the minimum loan is $25. For Tier 3, 4, 5 and 6 members, the minimum loan is $1,000. Our automated telephone system can let you know how much you are eligible to borrow, and the minimum payment amount, within 30 days of being eligible. Call 1-866-805-0990 (518-474-7736 in the Albany, New York area) and press 3 for member, then 1 or 2 (depending on which retirement system you are in, the Employees’ Retirement System or the Police and Fire Retirement System), then 1 for loan services.

Because IRS regulations affect the way we must calculate the taxability of retirement loans, some loans may be subject to federal taxes and an additional 10 percent penalty, depending on how much you borrow and under which loan type. We strongly advise you to speak to a customer service representative before taking a taxable loan.

  What date am I eligible for a loan?

Tier 1 and 2 members must wait 90 days from the date of their last loan. Tier 3, 4, 5 and 6 members must wait one year from the date of their last loan. Our automated telephone system can let you know your eligibility date. Call 1-866-805-0990 (518-474-7736 in the Albany, New York area) and press 3 for member, then 1 or 2 (depending on which retirement system you are in, the Employees’ Retirement System or the Police and Fire Retirement System), then 1 for loan services, and 3 for loan eligibility information.

  How do I apply?

  What’s the status of my loan?

Once your application has been received, our automated telephone system can confirm receipt at any time, day or night. Please call 1-866-805-0990 (518-474-7736 in the Albany, New York area) and press 3 for member, then 1 or 2 (depending on which retirement system you are in, the Employees’ Retirement System or the Police and Fire Retirement System), then 1 for loan services, and 4 for information about your recent loan application.

Member Annual Statement

  How do I get a copy of my Member Annual Statement?

  • Email us using our secure contact form.
  • Call toll-free, 1-866-805-0990 (or 518-474-7736 within the Albany, New York area).

We mail annual statements starting in May and generally ending in July. Your statement will be mailed to your employer if you’re on the payroll as of March 31. If not, we’ll mail it to your home address that we have on file at the end of the mailing cycle.

We can’t reprint:

  • Statements from previous years.
  • Reprints of the current year from March 31 until the mailing for that year is complete (generally early July).

Mortgage Letter/Account Verification

  I need account verification for a mortgage company.

Email your request using our secure contact form. Tell us what information you need (account balances or mortgage letters are most common). Be sure to include your personal verification and contact information when you submit your email. In most cases, we’ll mail you a letter within five to seven business days.

Direct Deposit

  What day are checks deposited?

Retirees/beneficiaries enrolled in our Direct Deposit Program have access to their retirement benefit on the last business day of the month (indicated by an “E” on the Pension Payment Calendar.) If you need to change your bank account information or cancel direct deposit, it is important to contact us before you make any changes to your bank account.

  How do I sign up for direct deposit?

To sign up for Direct Deposit, please complete a Direct Deposit Enrollment Application (RS6370) Adobe pdf. You must either attach a voided check, or have a bank representative complete Section 3 of the form. Enrolling in Direct Deposit is easy, and once you are enrolled, your retirement benefit will be electronically deposited into your checking or savings account on the last business day of each month.

  How do I change my direct deposit information?

To change your current Direct Deposit information within the same bank, you can either complete the Direct Deposit Enrollment Application (RS6370) Adobe pdf, or the bank can fax the following information to us on bank letterhead: your name, retirement or registration number, old and new routing and account numbers, and a bank representative’s signature. To change your direct deposit to a different bank, please complete a new Direct Deposit Enrollment Application (RS6370).

  How do I cancel direct deposit?

You must notify us in writing if you wish to cancel your direct deposit. Please mail or fax a letter stating your wish to cancel direct deposit and have your check mailed instead. Be sure to include your retirement number, name, address, and signature. We cannot accept an email. Your letter can be faxed to 518-473-5323, or mailed to:

Pension Checks

  What day are checks mailed?

Your retirement benefit check is mailed on the second-to-the-last business day of each month for that month (indicated by an “M” on the Pension Payment Calendar). If you have not received your retirement benefit check by the eighth of the following month, please contact us to request a replacement check. If you received your check and it was lost or stolen, contact us immediately. If you email us, be sure to include your telephone number (with area code) so we can call you.

  What do I do if I did not receive my check?

If you receive your check by mail, it is important to notify us of any address changes as quickly as possible as the Post Office may not forward your check.

If you choose to enroll in our Direct Deposit program, your retirement payments will be deposited directly into your checking or savings account on the last business day of each month. Over 90 percent of our retired members take advantage of this safe and convenient way to receive retirement benefits.

Pension Estimates for Employees’ Retirement System (ERS) Members

  I am within 5 years of retirement eligibility

Most ERS members who are age 50 or older can request a pension estimate (benefit projection) by phone at any time by using our automated system, as long as they have enough years of service to be eligible for a pension. The automated system will use salary reported to date, and project additional service credit based on a date of retirement up to five years in the future.

Call toll-free, 1-866-805-0990, press 3 for active members, 1 for ERS, 4 to receive a projection of your retirement benefits, and then follow the prompts. In most cases, these projections are mailed on the next business day to your home address.

  I am more than 5 years away from retirement eligibility

Contact us to request an estimate (only if you have enough years of service to be eligible for a pension). In most cases, we’ll mail your projection to your home address on the next business day. Even if you’re not near retirement age, getting an estimate may prove beneficial if you’re considering leaving the payroll of a participating employer.

Pension Verification Letters

  I need verification of my pension income and my Retiree Annual Statement isn’t acceptable proof.

Our retirees often need Pension Verification letters for housing or the Home Energy Assistance Program (HEAP). To request a pension verification letter:

Email your request using our secure contact form.

  • Tell us what information you need and be sure to include your personal verification and contact information when you submit the form.
  • In most cases, we’ll mail your letter in five to seven business days.

Fax your request to 518-473-5590.

  • Include your retirement or registration number, current address, signature and phone number in case we have questions.
  • Tell us whether you want the letter mailed or faxed to you (and provide a fax number).

If you want the letter sent to an entity like a mortgage company, they must fax a request to us on their letterhead along with a signed release from you giving us permission to release your information.

Service Credit

  How do I apply to receive credit for previous public employment?

You may be able to receive credit for service with a participating employer before you joined NYSLRS. To apply, please complete our form, Request for Previous Service (RS5042) Adobe pdf, and return it to:

  How do I apply for military service credit?

You may be entitled to receive additional credit toward your retirement benefit for your military service. Once we receive your request, we will determine if you qualify.

Withdrawing from NYSLRS

  How do I withdraw and receive a refund of my contributions?

You can withdraw and receive a contribution refund if you separate from service (terminated or resigned) with a participating employer and have less than 10 years of service credit.

Complete a Withdrawal Application (RS5014) Adobe pdf after you’ve been off the payroll for at least 15 days. Be sure to read and submit all four pages.

You can receive the refund directly or roll your contributions over to an Individual Retirement Account (IRA) or other qualified retirement plan.

1099-R reprint

  How do I get a reprint of my 1099-R tax form?

We mail all 1099-R forms by January 31 and generally accept reprint requests beginning the second week in February. To request a reprint:

  • Submit an automated request through our website.
