You can request reprints of your most recent 1099R forms by sending an automated request.
You have a choice of the 3 most recent calendar years. For additional years, please contact us. You must include your Name and Retirement Number for your request to be completed. Failure to provide all information will make it impossible to fulfill your request. Please note that reprint requests via the Internet can only be sent to the address on file with the Retirement System.
