Verification of Agency Return Address on Employee W-2 Statements

Purpose

To request verification of the agency address that will be used as the return address on employee W-2 statements for 2006.

Agency Actions

Agencies must review and confirm the agency’s address for all applicable Department IDs listed in the Control-D Report NPAY 529 (Agency W-2 Return Address Report) is correct.  Agencies must send an e-mail to [email protected] by November 28, 2006, confirming that the existing address is correct or indicating the address changes.  Please type “W-2 Return Reply” on the subject line of the e-mail.  Agencies must also include their five-digit Agency Code number(s) in the e-mail.

For agencies that cannot send e-mail, a fax must be sent by November 28, 2006 to (518) 486-3099, Attention:  Sue Curcio.

Note: This address information will be used only to print the agency return address on employee W-2s.

OSC Actions

OSC will update the Agency W-2 Return Address file with the information supplied.  A payroll bulletin will be issued at a later date requesting agencies to review a new Control-D report NPAY 529 to confirm updated information.

Response Deadline

Agencies must respond by November 28, 2006 to ensure that the correct return address is printed on employee W-2 statements for 2006.
