The purpose of this bulletin is to request verification of the agency address to be displayed on Employee Form W-2 for Tax Year 2021.
This bulletin is effective immediately.
Agencies must verify the agency return address for all Department ID(s) associated with their agency as listed in the Control-D Report NPAY529 (Agency’s Address Listing).
If the agency address is correct, no further action is needed.
If the agency address is not correct, agencies must submit corrections prior to November 19, 2021. Requests for agency name and address corrections may be directed to the Tax and Compliance mailbox. Email requests should indicate "Agency Return Address" in the Subject line, the five (5) digit Department ID, correct agency name, and correct address for each agency to be updated. Changes to the agency name field must be within the 30-character limit.
Note: Changes made to the agency return address for Form W-2 also changes the return address for direct deposit advice statements.
Agency Response Deadline
Agencies must submit corrections prior to November 19, 2021, to ensure the correct return address is printed on the Employee Form W-2 (Wage and Tax Statement) for 2021.
OSC will update PayServ with the agency name and address provided by agencies.
