To request verification of the agency address that will be used as the return address on employee W-2 statements for 2007.
Agencies must verify the return address listed for all their Department ID(s) in the Control-D Report NPAY529 (Agency’s Address Listing). The address on the NPAY529 is identical to the return address used for direct deposit advice statements. If the agency address is correct, no further action is needed.
If the agency address is not correct, agencies must send an e-mail to the Payroll Deductions mailbox providing the correct address prior to November 28, 2007. Please indicate “W-2 Return Reply” on the subject line and include your five-digit Agency Code number(s) along with the correct agency return address in the e-mail.
OSC will update the agency’s address file with the information provided.

Agencies who need to correct their address must respond prior to November 28, 2007 to ensure that the correct return address is printed on employee W-2 statements for 2007.
